HIPAA Policy for Verified Providers, LLC
Verified Providers, LLC enagages in the business of online forms creation and storage of Personal Health Information (PHI) for physicians who subscribe to our service. We do not enagage in the practice of medicine nor do we establish any type of doctor - patient relationship. Because we strive to keep all stored PHI secure from improper use, we have implemented a security policy that we believe meets or exceeds the "best effort" requirement as stated by the HIPAA privacy act.
Security measures we have implemented to secure your data:
All PHI is stored on a dedicated HIPAA compliant server based in Texas. Our hosting service, CI Host, is one of the the very few hosting companies who have implemented a comprehensive HIPAA security compliance policy. Please contact CI Host at www.cihost.com for more information regarding their compliance policy.
Username and Password: Each user is required to create a unique username and password. Verified Providers, LLC will not be held responsible for security breeches due to the sharing of usernames and passwords by subscribing members.
128k Bit Encryption: All PHI is encrypted under 128k Bit encryption using a secure socket layer (SSL) certificate.
All PHI is backed up on our proprietary servers located in various secure locations as contracted by Verified Providers, LLC.
Verified Providers, LLC will establish a Business Associates contract with all subscribing members. Under the HIPAA act, we are not considered a "covered entity" and therefore are not required to establish a HIPAA compliance policy. However, because we believe all PHI should be protected, we have implemented a HIPAA compliance policy as seen below.
The following HIPAA compliance policy was created to protect your PHI. The language in the following text was created for "covered entities" which we are not as defined by the HIPAA privacy act. The text was modified to fit our unique relationship with subscribing physicians and their patients.
